The North Carolina Alliance for effective Care Transitions (NC ACT) is a gathering of stakeholders representing hospitals, long term care, assisted living, home health, hospice, mental health, case management, insurance plans, community care networks, patients and others meeting to coordinate efforts improving care transitions across all settings for all North Carolinians.
Operating partners include: the North Carolina Quality Center, the NC Department of Aging and Adult Services, The Carolinas Center For Medical Excellence, Community Care of North Carolina and Carol Woods Retirement Community.
Person centered quality services and supports when North Carolinians transition from one healthcare setting to another.
NC ACT includes all stakeholders to: Connect services and resources; identify best practices and needs; and educate the public, providers, and decision makers on person centered quality services and supports for optimal healthcare transitions.
